Pre-Registration

Pre-registration is an important part of the surgery process at the Surgery Center of New England.  Knowing your health information and medical history helps ensure a safe, comfortable and simple visit. It is required and we ask that you complete pre-registration at least 7 days in advance of your procedure.

• For New patients the average time to complete the form is 25 – 30 minutes depending on your personal medical history.
• For returning patients the time will be less, but we ask that you make sure to review the whole form and update it with any changes.
• Upon completion, a staff member or nurse may call you for further clarification if necessary.

Information Needed

• Prescription medications – names and dosage of each you take.
• Over-the-counter medications – names and dosage of each one you take regularly.
• Vitamin and herbal medications – names and dosage of any that you take.
• Allergies/Sensitivities you have and the allergic reaction(s) they cause.
• Previous surgeries, and the year they were performed
• Medically Implanted Devices – the manufacturer and model # of any device you may have.
